Who you are contracting with

These terms are an agreement between you and Longevix MSO, LLC (“Longevix”, “we”, “us”), a management services organisation. Longevix provides technology, administrative and support services. It does not practise medicine, does not employ the clinicians who evaluate you, and does not dispense medication.

Clinical services are provided by independently licensed providers practising through an affiliated professional entity. Medications are dispensed by licensed pharmacies. Your relationship with those clinicians and pharmacies is separate from this agreement, and nothing here alters it.

Eligibility

You must be at least 18 years old and physically located in a state where the service is available. You must provide accurate information about your identity, medical history and current medications. Providing inaccurate information can lead to unsafe care, and it is grounds for termination.

You may not use the service on behalf of another person, and you may not resell, redistribute or transfer any medication dispensed to you.

What the service is, and is not

The service supports non-emergency care paid for directly by you rather than billed to insurance. It is not a substitute for emergency care. If you are experiencing a medical emergency, call 911 or go to your nearest emergency department.

Completing an intake does not entitle you to a prescription. A provider may determine that treatment is not appropriate for you, and that determination is clinical and final.

Membership and billing

Membership plans renew automatically at the interval you select until cancelled. By starting a plan you authorise recurring charges to your payment method. The specific authorisation terms are set out in the Payment & Billing Consent.

Prices may change. We will give notice before a change takes effect for an existing member, and a change never applies to a period you have already paid for.

You may cancel at any time from your account. Cancellation stops future billing. It does not retroactively refund a period already served or medication already dispensed.

Your account

You are responsible for keeping your login credentials confidential and for activity under your account. Tell us promptly if you believe your account has been accessed without your authorisation.

We may suspend or terminate an account for non-payment, for providing false information, for conduct that risks the safety of staff or other patients, or where required by law or by a clinician's judgement.

Intellectual property

The site, its content, and the Longevix name and marks are owned by us or our licensors. You may use the site for your own personal, non-commercial purposes. You may not scrape, republish or create derivative works from the content without written permission.

Your medical records are not covered by this section. Those are governed by the applicable privacy notices and by law.

Disclaimers and limits

The site and the administrative services are provided “as is”. We do not warrant that the site will be uninterrupted or error-free. This does not limit the professional obligations of any clinician or pharmacy, which we cannot and do not disclaim on their behalf.

To the maximum extent permitted by law, Longevix is not liable for indirect, incidental, special or consequential damages arising from your use of the site or the administrative services. Nothing in these terms limits liability for fraud, for personal injury caused by negligence, or for anything else that cannot lawfully be limited.

Changes

We may update these terms. The version in force is published here with its version date, and superseded versions remain available at a dated URL so it is always possible to establish which terms applied on a given date. Material changes will be notified to members before they take effect.
